This form is for those agencies who are submitting a nomination for a Foundation Housing property under a specific program. You must have the nomination rights to a specific property to complete this form (e.g. ILP, CDHP, CAP etc.).

Please note: Once the form is received a Foundation Housing coordinator will be in touch within two business days to discuss the nomination. As part of this process and prior to any accommodation being offered, we may ask the applicant to provide further documentation and evidence to support this application.

Foundation Housing’s – (FHL’s) aim is to sustain the tenancy and the information you provide will enable us to assess the suitability and risk of the proposed nominee, as well as the ability of FHL staff to safely and appropriately manage the tenancy.
Whilst we respect the client’s right not to disclose information, this information is critical to enable a thorough assessment of the supports needed to maintain a successful tenancy.
Pets are an important part of people’s lives, however not all FHL properties are suitable for pets. If the selected property is not appropriate for pets, we will advise you of other options once your application has been submitted.
If the pet is a therapy or support animal, then please upload evidence of this at the end of the nomination form.
Before moving into a property you will be asked to sign a legal-binding tenancy agreement. This is a digital process which is done through DocuSign and you will need an email address for this process.
Bonds are required for all new tenancies and must be paid prior to a tenant moving into their new home. Bonds are processed online, more information about this process is provided at the Sign Up meeting.
Bond assistance can be obtained through the Department of Communities for some tenants and this is explained further here.
